The following is a summary of “Barriers and Facilitators to the Implementation of a Health-Related Social Needs Screening and Referral Intervention in the Bronx,” published in the May 2025 issue of Journal of Primary Care & Community Health by George et al.
Researchers conducted a retrospective study to investigate the barriers and facilitators to health-related social needs (HRSNs) referrals and the integration of Community Health Workers (CHWs) within clinical teams.
They performed qualitative interviews with CHWs, Community Health Worker Institute (CHWI) program staff, and clinicians from ambulatory care clinics. Semi-structured interviews, based on the Consolidated Framework for Implementation Research, were audio-recorded, transcribed, and analyzed using rapid qualitative methods.
The results showed that clinicians supported the CHWI and referral program, but time constraints during patient visits limited effective screenings and referrals. The CHWs were recognized as valuable advocates but faced challenges due to unclear clinical roles among patients and clinicians. In some cases, particularly in the emergency department, fast-paced environments and unclear workflows further complicated the integration of CHWs. Hierarchical power dynamics between providers and CHWs contributed to this confusion. Clinics with strong leadership, defined roles, and proper site preparation demonstrated better integration of CHWs.
Investigators concluded that CHWs play a vital role in addressing HRSNs, but successful integration into clinical teams depends on overcoming logistical and educational barriers.
