Patient Harm Escapes Database Inclusion

A report from the Office of Inspector General indicates that approximately 86% of adverse events in hospitals fail to make it into databases designed to improve quality of care. The report, available at http://oig.hhs.gov, adds that 62% of events are not captured because staff felt they were not reportable.
